Case Management Specialist at BCFS Health & Human Services - Washington D.C.

BCFS Health and Human Service is hiring a bilingual Case Management Specialist for our Post Release & Home Study (PRHS) program.  BCFS is a system of health and human services organizations operating across the U.S. and internationally.  We offer a wide variety of programs including foster care & adoption, support for survivors of human trafficking and early childhood education, among many others.  The PRHS program serves unaccompanied immigrant minors who are primarily Central American youth that have been placed with a family member or sponsor while they go through the immigration legal process.  Case managers help the minors integrate into their new home, school and community by conducting ongoing safety and well-being assessments and connecting the family to critical community based resources.  BCFS HHS is the largest provider of post release services for this clientele, with 15 offices throughout the country.  The position based in Washington, D.C. will involve extensive travel to clients’ homes throughout the Northeast U.S., with the goal of helping the families maintain a safe and healthy placement.
